WSI Independent Exercise Program Log's

The WSI Independent Exercise Program Log is a way for people who have been injured on the job to have access to the facility and have WSI cover their bill.  In order for them to have access to the facility they need to fill out a membership application and be set up in Daxko by Jennifer Keller.  

They will ask us to sign and date this log each time they visit.  We also need to make sure they are an active member in Daxko.  Don't just sign the paper and let them in.
