Daily Staff Arrival Questionaire

Hey everyone,

As discussed in our safety training video, our Daily Staff Arrival Questionnaires and thermometer will be located in the copy room behind the East Member Services Desk.  This is a requirement for everyone when they come in to do the temperature screening as well as filling out the sheet.  Once the sheet is completed please put it in my mailbox.  

Reminder for the thermometer:

Press the button once to turn on.  You should see ---F.  Hold scanning section 1 inch from your forehead and press and hold the button.  The thermometer will vibrate once your temperature is read and display the results above the button.  You do not need to press anything to turn it off.
