Financial Assistance Applications

Hey all,
We need to get better at checking over Financial Assistance applications that are coming in. There have been a lot of them lately that have been turned in without any financial documentation. We CANNOT process an application without this documentation, therefore we cannot accept an application without that documentation.
If they do not bring in documentation with their application please go through what is needed from them in order to proceed. We will not process the application without proper documentation. In the case of someone claiming to have absolutely nothing for income please walk through all of the different forms with them. If they still claim to have nothing, put the application in my mailbox and I will follow up with them. Please make some notes about your conversation with them so I have an idea on their situation and attach it to the application.
On the back page of the application is a breakdown of their income. If they have anything written down in any of those 8 areas we need documentation for it.
Below is a breakdown of documentation/forms that may be needed for each of the 8 categories.
Salary, Wages, and Tips: Two most recent pay stubs, copy of their most recent year's tax return (Form 1040 or 1040EZ), a letter from their employer stating their annual salary.
Unemployment Compensation: Documentation from the government stating how much they receive.
Social Security Compensation: Documentation from the Social Security Administration stating how much they receive.
Child Support: Any document that states how much they receive each month
Government Financial Assistance: This can be a wide variety of things including Housing Assistance, Food Stamps, TANF (Temporary Assistance for Needy Families), or Medicaid.
401 (k) Retirement: A document stating their payouts from any retirement savings or investments they receive on a monthly basis.
School Loan Income: Amount they receive in school loans with their tuition bill for the semester to see what is left over from the loan.
Other: Any other income that does not fall into the categories above.
