Taking payment for on site flu shots

For the upcoming flu shot offering for employees there is an option for individuals to pay out of pocket for their shot.  To do this they will be stopping at the Member Services desk.  The flu shot is listed in the One Time Fee area as "Flu Shot".  After taking payment we have to give them a print copy of the receipt to attach to the "Company Bill" form they need to bring to their appointment. The "Company Bill" form was attached to the Paycom email that was sent out to all employees.  They will need to print out the form in advance or get one from the supervisor to complete before their appointment. 

Please remember, the flu shot is only being offered to YMCA and Family Wellness employees.  Family member of staff are not able to register for the flu shot through us at this time, nor is it available to the general public.
