New DoD (Department of Defense) Membership Forms

Hey all,

I know these memberships are super rare but I just wanted to put this information on here to be a reference for you all.  We received new DoD Membership Forms.  I have attached a digital copy of the new version for you to scroll through.  The new forms have been printed out and replaced the old ones in the drawers at both East and West desk. The biggest things to remember with these types of memberships are listed below:

• • The Eligibility Form on page 6 needs to be filled out by everyone.

  • Page 8 needs to be filled out for Independent Duty Personnel (individuals that are not being deployed, usually military recruiters).  Their instructions are on page 7.

  • Jennifer Keller fills out the information on page 9 and sends that in to the government. 

• If the individual turning in the paperwork isn't sure if they are IDP make sure that they have the Eligibility Form on page 6 completed and get a good email address for them and send it through to Jennifer Keller.  If she needs any more information/documentation from them she will reach out to them. 

  • We set up their membership in Daxko.  They will get a Household - Locker Room Semi Annual membership with Child Watch for the appropriate number of children on the account.  When you get to the payment screen add the discount group DoD Program.  This should bring the balance to zero.
