Personal Training Interest Forms

Just want to clarify that the Personal Training Interest Forms are for individuals that have not previously done personal training with us or for new members.  We do not need to have existing personal training clients fill these forms out because they already have a trainer.  

An easy way to find out if someone has done personal training before is to simply ask.  "Have you done personal training with us before?"  If yes, skip the form and ask them how its been going, who they've been working with, what their goals are, etc.  If no, have them fill out the form.  If they are new, DO NOT recommend one trainer over another!
