Financial Assistance Application Reminders

Hey team,

Please remember that when we are accepting financial assistance applications that are being dropped off that we are checking to see if they have proper financial documentation.  If we do not have proper financial documentation we cannot process an application.  Documentation options are below:

- Two most recent paystubs

- Most recent years tax return (1040)

- Financial assistance received from the government (food stamps, housing assistance, social security, disability, unemployment, child support, etc)

- Letter from partner organization stating their involvement with them (Burleigh/Morton County Social Services, PATH, West Central Human Services, South Central High School, Youthworks, or United Way Homeless Shelter)

Additionally, when someone drops off an application please make sure you are putting the date that it was dropped off and ask them/write down their preferred method of being contacted (email or phone) on the application.  This helps Samantha out a ton when she is processing the applications so she knows which was received first and how they wish to be contacted so they can get in here as soon as possible to start working on their wellness journey.
