Financial Assistance Application

Team,

Please do not accept financial assistance application with financial documentation. Without documentation they will not be processed and cannot be approved. Any financial assistance provided by the government (housing assistance, unemployment, SNAP, Medicaid, SSI, etc.) is considered income verification for us because individuals would not be receiving those assistance packages from the government if they did not me their qualifications. Individuals receiving those services are able to request documentation of how much they receive. 

Below is a breakdown of the types of documentation and forms of income verification that may be needed for each of the 8 categories.

Salary, Wages, and Tips:  Two most recent pay stubs, copy of their most recent year's tax return (Form 1040 or 1040EZ), a letter from their employer stating their annual salary.

Unemployment Compensation: Documentation from the government stating how much they receive.

Social Security Compensation: Documentation from the Social Security Administration stating how much they receive.

Child Support:  Any document that states how much they receive each month

Government Financial Assistance:  This can be a wide variety of things including Housing Assistance, Food Stamps, TANF (Temporary Assistance for Needy Families), or Medicaid.  

401 (k) Retirement: A document stating their payouts from any retirement savings or investments they receive on a monthly basis.

School Loan Income:  Amount they receive in school loans with their tuition bill for the semester to see what is left over from the loan. 

Other:  Any other income that does not fall into the categories above.

There are a few organizations that we accept documentation from to help their participants including West Central Human Services (will supply a Release of Information waiver), Blessed Builders (will supply a letterhead from Blessed Builders with application), South Central High School (teacher will be with students to fill out application), PATH (will supply letterhead from PATH with application), Youthworks (will provide letter with application), United Way Homeless Shelter (will provide letter with application).
